Companion Care for Life

Have you made plans for the ongoing care of your animals? Our Companion Care for Life Program (CCLP) helps guardians walk through the steps necessary to ensure your pets are cared for in the event of your passing or inability to provide care due to age, condition or illness.

We know that it is not always possible to make arrangements with family members or friends to care for your pets. You may designate through your will or trust that your animals be put into the care of the Humane Society of Boulder Valley.

How to Enroll

Work with your estate planning attorney to draw up a will or trust for you.Ā  You may also useĀ FreeWill, a free estate planning resource,Ā to get started.

In your will, name family or friends or the Humane Society of Boulder Valley to take your pets upon your permanent incapacity or passing. The following is suggested language for a will or a trust concerning a pet:

I give any pet(s) that I may own at the time of my death to the Humane Society of Boulder Valley. I authorize the Humane Society of Boulder Valley to place my pet(s) in a new home. To arrange for intake of my pet(s), my trustee/executor shall contact the Humane Society of Boulder Valley at 303.442.4030.”

Complete the Companion Care for Life form for each of your animals.Ā The information you provideĀ on these formsĀ will help HSBV place your pets in the best possible new home. This information can be updated at any time to reflect changes in animal diet, health, habits or other items.

Keep a copy of the Companion Care for Life formĀ with your will or trust and inform your executor or trustee of the program. Please also send a copy of the forms to Francie Wojcik at Francie.Wojcik@boulderhumane.org.
